EMP|SFM is pleased to support the fund-raising efforts of fellow 501(c)(3) organizations. To obtain four (4) guest passes for use at your next fund-raising event fill out the form and your request will be processed within 30 days. The following policy restrictions are in place regarding this support:

• All EMP|SFM donation requests must be submitted via the web form.
• EMP|SFM offers four (4) guest passes ($60.00 value) for each request.
• Please limit your requests to one per organization, per year.
• EMP|SFM donates passes only to 501(c)(3) organizations with valid Federal Tax ID numbers.
• EMP|SFM passes are good for one year from date of issue.
